非浸潤型副鼻腔アスペルギルス感染症により視神経症を呈した1例

要約　目的：真菌による眼窩先端部症候群は重篤な転帰をたどることがある。画像所見と採血所見が乏しく，診断に苦慮した副鼻腔アスペルギルス感染症の1例を報告する。

症例：64歳，男性。左眼視力低下で受診した。矯正視力は右1.2，左0.02。視神経乳頭に軽度腫脹を認め，左相対的瞳孔求心路障害が陽性であった。眼球運動時痛と眼球運動障害はなかった。造影MRIにて視神経管に造影効果および蝶形骨洞の粘膜肥厚を認めた。採血にてβ-Dグルカン上昇などの明らかな異常所見を認めず，真菌感染症は考えにくいとの判断でステロイドパルスを施行した。1クール終了時に乳頭腫脹は軽快し，矯正視力と自覚症状は改善したが，2クール終了時に自覚症状が悪化した。画像検査にて蝶形骨洞の肥厚した骨皮質と左視神経管内への炎症の波及が認められ，真菌感染症が疑われた。CTでは骨破壊像はなかった。副鼻腔生検を施行したところ，篩骨洞内の上方から天蓋部にかけてアスペルギルス塊を認め，副鼻腔アスペルギルス症による眼窩先端症候群の診断に至った。

結論：本症例の副鼻腔アスペルギルス症は骨破壊のない非浸潤型で，左視神経管内への炎症波及による視神経症と考えられた。画像所見と採血結果において，副鼻腔真菌症を積極的に示さなかったことが診断を困難にしていた。副鼻腔真菌症では，β-Dグルカンの測定が有用とされているが必発ではなく，臨床的に副鼻腔真菌症を疑った際は早期の生検が診断に有用であると考えられた。
