20年以上経過観察できた膠様滴状角膜ジストロフィの6症例

要約　目的：膠様滴状角膜ジストロフィ（GDLD）は，常染色体劣性の遺伝性疾患であり，アミロイド沈着による角膜混濁をきたす。今回，筆者らは長期間加療・経過観察した6症例を経験したので報告する。

対象と方法：順天堂医院眼科でGDLDと診断し，20年以上経過観察が可能であった6例12眼を対象とし，レトロスペクティブに評価した。評価項目は初診時および最終受診時の平均年齢・視力，観察期間，角膜移植回数，重症度である。重症度分類は「角膜難病の標準的診断法および治療法の確立を目指した調査研究」のワーキンググループで定義した分類を使用した。

結果：対象は6例12眼（男性：4例8眼，女性：2例4眼），初診時年齢は24.0±7.5歳（14〜33歳），平均視力は0.02（logMAR視力1.70）であった。観察期間は39.8±7.3年（28〜48年），角膜移植回数は片眼4.3±2.2回（2〜9回），最終受診時の平均年齢は65.5±10.1歳（47〜77歳），平均視力は0.013（logMAR視力1.89），2眼は光覚消失となった。光覚消失の原因は，網膜剝離1眼，緑内障1眼であった。重症度はⅡ度1例，Ⅲ度2例，Ⅳ度3例であった。

結論：20年以上の長期に経過を追えたGDLDの臨床経過を報告した。GDLDは，初診時視力が不良の症例が多かった。若年発症であり，長期間の視力改善は認められず，角膜移植を繰り返した。
