角膜内皮移植後の移植片接着不良に対しROCK阻害薬の有効性が示唆された2例

要約　緒言：角膜内皮移植術（DSAEK）後の移植片接着不良に対しROCK阻害薬である0.4％リパスジル塩酸塩水和物（リパスジル）点眼を2回/日にて追加した。その後，外科的追加処置なしで良好な移植片接着となった症例を2例経験した。

症例：症例1は65歳，女性。フックス角膜内皮ジストロフィおよび落屑緑内障により内皮減少を認め，近医で白内障術後に水疱性角膜症（BK）を発症，川崎医科大学附属病院（当院）でDSAEKを施行した。DSAEK直後に空気瞳孔ブロックを起こしたため空気を抜去した。移植片接着不良を生じたため，空気再注入術を予定していたが，リパスジル点眼を行い36時間後に移植片は自然接着を認めた。症例2は75歳，男性。落屑緑内障に対し当院で複数回緑内障手術を施行した後，BKを発症したためDSAEKを施行した。線維柱帯切除後の低眼圧が一因となり移植片接着不良が遷延した。空気や10％SF₆ガスを注入するも移植片は接着せず。再移植予定としていたが，リパスジル点眼を投与した1か月後に移植片は自然接着を認めた。

考按：今回，DSAEK後の移植片接着不良に対しROCK阻害薬であるリパスジル点眼が有効であった可能性のある2例を経験した。作用機序は，ROCK阻害によって内皮細胞脱落部への創傷被覆が促進しバリア機能およびポンプ機能が回復したためと推察される。ROCK阻害薬は角膜内皮移植術後の移植片接着不良に対し有用である可能性がある。
