両眼に梅毒性視神経網膜炎をきたした後天性梅毒の1例

要約　背景：梅毒による眼感染症（ocular syphilis）は多彩な所見を呈し，早期の診断は難しい。今回，後天性梅毒の初診時所見が両眼視神経乳頭浮腫であった症例を経験したので報告する。

症例：35歳，男性。約2か月前からの左眼羞明感を主訴に当科を初診となった。矯正視力は右1.5，左1.2。前眼部に特記所見なし。眼底検査で両眼視神経乳頭浮腫と左眼黄斑部漿液性網膜剝離があった。光干渉断層計検査（OCT）では網膜色素上皮の不整などはみられなかったが，硝子体腔内にわずかに点状高信号がみられた。視野検査で両側マリオット盲点の拡大を認めた。頭部画像検査で異常はなかった。初診時の血圧は160/90mmHgであった。フルオレセイン蛍光眼底造影検査（FA）で，両眼視神経乳頭および乳頭周囲血管，左眼周辺網膜血管から蛍光漏出がみられた。炎症性疾患を疑い血液検査を行ったところ，梅毒抗体が強陽性であった。髄液にも梅毒抗体陽性が証明された。梅毒感染に伴う視神経網膜炎と診断し，神経梅毒の治療に準じてベンジルペニシリン1800万単位/日を2週間点滴した。治療後，自覚症状と乳頭所見は軽快し，治療7か月半後のFAで乳頭からの漏出所見は消失した。

結論：Ocular syphilisは梅毒感染の第2期以降でみられる。OCT所見では特徴的な網膜外層や網膜色素上皮異常がみられると報告されているが，発症初期では乳頭浮腫のみで典型的なOCT所見を示さない例があり，診断にあたっては注意を要する。
