ステロイドパルス療法を施行した強膜炎の2例

要約　目的：強膜炎の約30％には関節リウマチなど全身性の自己免疫疾患を伴うことが知られており，原疾患がわかればそれに準じて治療を行うことになる。しかし，実際には強膜炎の約50％は原疾患が不明で，なかには治療に難渋する症例もある。今回，原因不明の強膜炎に対してステロイドパルス療法を行った2例を経験したので報告する。

症例：症例1は後部強膜炎で，ステロイドパルス療法への反応が良好で早期に寛解が得られた。症例2は眼科手術を契機として発症した壊死性強膜炎で，免疫抑制薬の内服・点眼に加えてステロイドパルス療法を行うも角膜真菌感染を生じ，最終的に角膜穿孔のため眼球摘出に至った。

結論：重症な強膜炎の治療選択は，明確な診断基準や治療指針がないため，病状経過や診察医師の経験で判断せざるをえないことが問題である。難治症例もあるため，治療開始時点から強力な治療が必要となる場合もある。
