進行性網膜外層壊死の僚眼にサイトメガロウイルス網膜炎を発症した1例

要約　目的：腎移植後の片眼に水痘帯状疱疹ウイルス（VZV）による進行性網膜外層壊死（PORN）を発症し，その後僚眼にサイトメガロウイルス（CMV）網膜炎を発症した1例を経験したので報告する。

症例：48歳，男性。腎移植後の拒絶反応抑制のため免疫抑制薬を内服していた。2010年12月に帯状疱疹が出現し，アシクロビル点滴によって改善した。その後，右眼の視力低下を自覚し，当院に紹介された。矯正視力は右0.15，左1.2であった。右ぶどう膜炎を認めた。前房水PCR検査でVZVによるPORNと診断した。アシクロビルおよびステロイド全身投与を開始し，2011年1月に右眼に対し硝子体手術，水晶体切除術，シリコーンオイル注入術を行った。同年11月に右眼に対し硝子体手術とシリコーンオイル再注入術を施行し，その後経過観察されていた。2019年10月に左眼の視力低下を自覚した。網膜中心静脈閉塞症による黄斑浮腫を認め，抗血管内皮増殖因子薬硝子体注射を行った。同年12月に左眼に硝子体混濁と網膜滲出性病変が出現した。PORNを疑い，左眼に対し硝子体手術とシリコーンオイル注入術を施行した。硝子体液からCMVが検出され，CMV網膜炎と診断した。現在，矯正視力は右0.07，左0.7である。

結論：免疫抑制薬内服中の患者では両眼にそれぞれ異なるウイルスによる網膜炎を生じることがあり，その都度しっかりした原因検索が必要である。
