角膜内皮移植後4か月で角膜浮腫をきたした1例

症例

患者：79歳，男性

主訴：右眼のかすみ

既往歴・家族歴：特記事項なし

現病歴：前々医に右眼の角膜浮腫を指摘され，前医へ紹介された。角膜ヘルペス疑いで加療を受けたが角膜浮腫の改善は乏しく，角膜内皮移植（Descemet stripping automated endothelial keratoplasty：DSAEK）を施行された。術後，右眼圧は25〜29mmHgと若干の高値で推移していた。角膜は透明化していたが，術後4か月で角膜浮腫が再燃した。拒絶反応の疑いでステロイド点眼および局所注射が行われたが改善せず，小円形に分布する角膜後面沈着物（keratic precipitate：KP）が出現したため精査目的に当院に紹介され初診となった。
