10代女性の点状脈絡膜内層症の1例

要約　目的：10代女性に発症した点状脈絡膜内層症（PIC）の症例を経験したので報告する。

症例：患者は17歳，女性。1週間前からの右眼視力低下を主訴に近医を受診し，精査目的に天理よろづ相談所病院を紹介受診した。

所見：視力は右（0.5p），左（1.5）と右眼の視力低下を認めた。右眼底には黄斑部に多発する黄白色病変と傍中心窩に網膜出血を認めた。脈絡膜新生血管（CNV）を伴うPICと診断し，プレドニゾロン15mg/日の経口投与を開始した。同時にCNVに対してアフリベルセプト硝子体注射を施行し，1か月ごとに計3回施行した。右眼視力は治療開始1か月後で（0.9），2か月後に（1.0）と改善傾向を認めた。治療開始から5か月後の現在，右眼視力は（1.0）を維持し，黄白色点状病巣は残存しているが，一部は色素沈着を伴い瘢痕化を認めている。

結論：今回，比較的若年でCNVによる出血を契機に発見されたPICの症例を経験した。PICは小児から中高年まで生じうる，視力低下をきたす疾患の1つとして注意が必要である。
