視神経炎が疑われその鑑別にen face OCTが有用であった傍中心窩急性中間層黄斑症の1例

要約　目的：傍中心窩急性中間層黄斑症（PAMM）は傍中心窩に網膜の白濁をきたし，急性の視野障害を伴う疾患である。今回，視神経炎が疑われたPAMMの1例を経験したので報告する。

症例：74歳，男性。1週間前から左眼の急激な霧視と視野障害を自覚し近医を受診，左眼視神経炎疑いで川崎医科大学附属病院を紹介され受診した。視力は，右（1.5），左（0.9），眼圧は，右12mmHg，左10mmHg，限界フリッカ値（CFF）は，右40Hz，左28Hzであった。相対的瞳孔求心路障害（RAPD）は左陽性で，RAPDの定量的評価が可能なRAPDx^®によるRAPD振幅は1.33（log units）であった。眼底では，左眼の視神経乳頭耳側に軟性白斑，中心窩周囲の網膜白濁がみられた。左眼光干渉断層計（OCT）では内顆粒層の高輝度所見が多発的にみられた。2日後，左視力は（0.4）と低下し，ゴールドマン視野では中心および傍中心暗点をきたした。左眼en face OCTでは，表層と深層に斑状の高反射所見が明瞭に観察された。以上の結果，左眼PAMMと診断した。en face OCTの高反射所見は経時的に消失し，6週後には左視力は（0.9），RAPD振幅0.03，CFFは39Hzと視機能も改善した。

結論：PAMM単独発症では，RAPD陽性やCFF低下を伴い視神経炎に類似した所見をきたすことがあり，その鑑別および経過観察にはRAPDx^®とen face OCTが有用である。
