典型的検眼鏡的視神経所見を欠いたレーベル遺伝性視神経症の1例

要約　目的：典型的な眼底所見に乏しく，診断に苦慮したレーベル遺伝性視神経症（LHON）の1例を経験したので報告する。

症例：48歳の男性が左眼の急速な視力低下を自覚し前医を受診した。前医の検査では明確な原因は特定されなかったが，その後右眼の視力も低下したため横浜市立大学附属病院（当院）を紹介され受診した。患者の家族歴に眼疾患はなく，また患者は30年近い喫煙歴を有していた。

所見：当院初診時の検査では，両眼で限界フリッカ値が低下していることが確認された。しかしながら，対光反応に異常はなく，眼底検査，血液検査，頭部MRI検査でも異常はなかった。造影剤アレルギーのため，蛍光眼底造影検査は行うことができなかった。臨床経過から視神経症の可能性が疑われたためLHON遺伝子検査を施行したところ，ミトコンドリアDNA14484番の変異が検出されLHONの診断が確定した。

結論：LHONは通常，急性期に視神経の検眼鏡的異常所見を呈することが一般的であるが，本症例では急性期から一貫してそのような異常所見はなかった。このような場合でも，中年男性で両側の視力低下が認められる場合はLHONを考慮すべきであると考えられた。
