小眼球を伴わないuveal effusion syndromeに対し硝子体手術を施行し網膜復位が得られた1例

要約　目的：小眼球を伴わないuveal effusion syndrome（UES）に対し硝子体手術単独で網膜復位が得られた1例を経験したので報告する。

症例：72歳，女性。1週間前より右視力低下を自覚し，大阪ろうさい病院初診時の視力は右0.07（矯正不能）であった。隅角形成不全に伴う緑内障に対し2年前に両眼に線維柱帯切除術を施行されていた。右後眼部は黄斑剝離を伴う下方の可動性に富む胞状網膜剝離と鼻側には脈絡膜剝離を認め，裂孔は確認できなかったため，小眼球を伴わないUESと診断し，硝子体手術を施行した。緑内障があり，結膜を温存するために強膜開窓術は併施しなかった。術後網膜復位が確認され，その後1年以上滲出液の再貯留を認めておらず，最終視力は右（0.6）であった。

結論：強膜開窓術の施行がためらわれるような真性小眼球を伴わないUESに対しては，初回治療として硝子体手術のみを施行することも選択肢の1つとなりうる。
