ロングチューブシャント手術の適応

ここが変わった！

以前の常識

・ロングチューブシャント手術は，複数回のトラベクレクトミーが無効であった症例が適応であり，緑内障手術の最終手段としての立ち位置にある．

現在の常識

・初回のトラベクレクトミーが無効であった場合の次の一手としてロングチューブシャント手術が適応となりうる．

・トラベクレクトミー術後の処置（laser suture lysisやneedling，眼球マッサージなど）や定期的な通院などが困難であり，適切な濾過胞の管理が難しいと考えられる症例にも適応となる．
