ハプティクスを毛様溝に確実に縫着した眼内レンズ毛様溝縫着術の術後長期経過

要約　目的：ハプティクスが毛様溝に確実に固定された眼内レンズ（IOL）毛様溝縫着術の術後長期経過を，過去のIOL縫着術の長期経過報告との比較を交えて報告する。

対象と方法：エンドスコープを用いてハプティクスが毛様溝に確実に固定されたことを術中確認したIOL毛様溝縫着術において，術後1〜14年6か月（平均56か月）観察した146眼を対象とした。この手術を必要とした理由（A），術後10年までの矯正視力経過（B）と最終観察視力（C），術後屈折誤差（D），前房深度（E），術後合併症（F）を検討した。

結果：Aは，白内障術後のIOL偏位が最も多く38.4％，次いで白内障手術合併症のためIOL非挿入27.4％であった。Bは術後8年まで術前より有意に良好で，CはlogMAR 0.15で過去の報告より良好であった。Dは−0.71±0.75D（平均±標準偏差）と近視化を認めた。Fは，術後硝子体出血24.0％，縫着糸の露出19.2％，瞳孔偏位18.5％，虹彩捕獲6.2％，縫着糸の断裂とIOL偏位は0％（過去の報告では，6〜28％）であった。硝子体出血は術後1か月以内に全例消退した。瞳孔偏位例の平均視力は，全例でのそれと有意差はなかった。

結論：本術式では，術後長期に良好な視力とIOLの安定的な固定が得られ，ハプティクスの毛様溝への正しい固定の重要性が示唆された。
