帯状疱疹を契機に慢性移植片対宿主病による重症角結膜炎をきたした1例

要約　目的：眼は造血幹細胞移植後の移植片対宿主病（GVHD）の標的器官の1つであり，視機能低下をもたらすことがある。近年，ヘルペスウイルス感染が慢性GVHDを発症，増悪させる可能性が指摘されている。今回，帯状疱疹発症後に慢性GVHDによる角結膜炎をきたした症例を経験した。

症例：51歳，男性。3年前に混合型急性白血病に対し造血幹細胞移植を施行後，原疾患は寛解を維持しており，1年前に免疫抑制療法は終了されていた。初診20日前に左上半身帯状疱疹を発症し，その10日後より両眼の充血，疼痛を自覚した。初診時に両眼に著明な結膜充血，浮腫性変化，偽膜形成を認め，右眼は角膜潰瘍をきたしていた。経過から慢性眼GVHDによる角結膜炎と診断しステロイド点眼加療を開始したが軽快せず，ステロイド内服，免疫抑制薬点眼，トリアムシノロン結膜下注射を行い眼所見は改善した。結膜組織の病理像では間質の線維化を認め，慢性GVHDに伴う結膜炎として矛盾しない所見であった。

結論：帯状疱疹を契機に眼慢性GVHDの症状が増悪することがある。慢性GVHDは全身性の病態であり，その治療には血液内科医との連携が重要と考えられた。
