漿液性網膜剝離によって診断された再発性多発軟骨炎

要約　背景：再発性多発軟骨炎は多臓器にわたる慢性再発性の炎症が特徴であるが，漿液性網膜剝離を合併した報告は少ない。漿液性網膜剝離による眼科受診を契機に診断に至った再発性多発軟骨炎の症例を経験したので報告する。

症例：84歳，女性。右眼の急激な視力低下を主訴に東京医科歯科大学病院に紹介され初診した。視力は右光覚なし，左矯正0.5，眼圧は右7mmHg，左16mmHgであった。右眼の強膜充血，角膜周辺部の菲薄化を認めた。前房は両眼深く，右眼には炎症細胞とフレアがそれぞれ2＋あった。右水晶体は強い核硬化と後囊混濁がみられた。超音波Bモードにより，網膜剝離，脈絡膜肥厚がみられた。顔面の視診にて鞍鼻を認め，再発性多発軟骨炎を疑い全身検索を行った。胸部単純CTでは，呼気時の気管虚脱を認めた。耳鼻科領域においては鼻咽頭ファイバースコープで鼻中隔穿孔がみられた。Damianiらの診断基準のうち3項目を満たしたため再発性多発軟骨炎の診断となり，ステロイドパルス療法およびアダリムマブ投与を行った。

結論：漿液性網膜剝離を伴う強膜炎の鑑別に再発性多発軟骨炎を挙げる必要性が示唆された。また，強膜炎の診断の際には顔面・耳介の視診が重要であることが改めて示唆された。
