終末期ケアにおける意思決定の事例からの考察

筆者は平成元年からがんの終末期ケアに取り組みを開始し，同9年に中国地方で初めての緩和ケア病棟を単独型として開設．その責任者として「がんの緩和ケア」に従事した後，それを平成13年12月に閉鎖（その理由については本稿の目的ではないので別稿に譲る． 詳細は当院ホームページ，http://www.kato-namiki.or.jp/kato-Saga050827/katosaga050827-1.html，佐賀緩和ケア研究会講演を参照）．今は，ジェネラリストとして19床の有床診療所を舞台に，がんであれその他の疾患であれ，在宅ケアを必要とする人たちを主な対象として終末期ケアを提供している（図）．また，その一方で，平成3年に設立されて現在も活動を続ける「緩和医療研究会」の事務局として，岡山大学をはじめとしたいくつかの医学部の卒前卒後における緩和医療教育の充実にも取り組んできた．

　本稿では，このような筆者の経験を通じて，実際に遭遇したいくつかの事例をもとに，「終末期ケアをめぐる意思決定のあり方」について私見を述べる．
