医療は，われわれ教師が，学生，研修医，医師を適正に教育できる能力をしのぐ勢いで発展している．バイオ医療の情報量は，教科書が捕捉できないことがらはいうまでもなく，一人の頭脳で捉えられる範囲を超えて急速に拡大している．技術が複合して発展したおかげで生活の質が改善され寿命が延びているが，技術を習得するための訓練も専門分化し習得に必要な時間も長くなっている．働く時間の制限から，教育に費やす時間にも限りがある．

　結果として医療システムは，Institute of Medicine（IOM：米国医学研究所）のレポート，“Crossing the Quality Chasm”（「質の断層への橋渡し」）が提示した原則に沿う形でのケアを提供することが，ますます困難になってきている．その原則とは，安全で，効率がよく，効果的でかつ衡平で時宜にかない，患者中心のケアというものである^1）．
