米国におけるPhysician Assistantの役割と日本における外科医療の分業化

　外科手術件数は年々増加し^1），また医療技術の進歩で手術の難易度も高まっている．一方，第一線で外科医療を支える40代後半から50代の医師（全体の4割を占める^2））は10年後には引退し，若い世代の医師は専門領域を自由に選択できる状況下で，訴訟リスクが高く労働条件の厳しい外科を希望する者が減少している．今後外科医の不足は急速に深刻化し，医学部の定員増だけでは効果的な対策とはならないであろう．

　筆者（遠藤）は患者として日米で手術を受け，外科医療を体験した．そのうえで行った米国および日本の医療現場の取材を通して，日本の外科医不足には苛酷な労働環境と専門外の業務負担が多いこと，一方米国にはそれを支援する分業体制と外科医を補佐する多くの専門職が存在し，彼等が重要な役割を果たしていることがわかった．中でもPhysician Assistant（以下，PA）は医師の監督下で医療行為を行い，入院から手術，集中治療・病棟管理，外来診察までを一貫して担うことで，外科医を支援する職種として不可欠な存在であった．本稿では現地調査をもとに，PAと医師の業務分担，代替・補完関係について述べ，日本における外科医療の分業化について考察する．
