英国の高齢者終末期ケアの動向①―「終末期ケア戦略」に基づいて進む国

　今月からは英国の終末期ケアについて紹介する．英国というと「ホスピス発祥の地」ということから終末期ケアが充実しているイメージがあるが必ずしもそうではない．例えば，右の図は，2008年における全人口ならびに65歳以上の高齢者の死亡場所の割合を示したものであるが，この図からもわかるとおり，英国民の死亡場所を見てみると，国民の半数以上が住み慣れた自宅で亡くなりたいと考えているにもかかわらず，実際には7割近くの者が病院で亡くなっており，自宅や施設で亡くなる者は全体の2割強程度に留まっている．また，終末期ケアの内容についても，質の高い終末期ケアを受けられているのはほんの一部に過ぎず，多くの者が不必要な苦痛を被り個人の尊厳を敬うようなケアを受けることができていないことが報告されている^1）．

　このため，英国でも，終末期ケアの充実や向上は重要な課題の1つとなっており，2000年頃から様々な終末期ケア施策が展開されてきている．中でも，2008年には，先進諸国の中でも初めてと言われている終末期ケアに関する国家戦略「終末期ケア戦略（End of Life Care Strategy）」を策定して，終末期ケアの充実・向上を図ろうとしている．
