英国家庭医から見た患者中心の医療

　医療が日々進化する現在，医療サービスにおける患者の要求水準は年を追うごとに高くなっている．需要者である患者のニーズが多様化・高度化する中で，患者が主体的に自分自身の医療にどう関わるのか，そして満足度の高い医療を国はどう構築し，どう実現していくのか．これは日本だけではなく，国際的に見ても重要なテーマだ．

　本稿では，日本から遠く離れた英国での取り組みを紹介したい．近年における英国の国営医療サービス（National Health Service，以下，NHS）の歩みには目を見張るものがある．1990年代のサッチャー政権下，長い待機時間，院内感染，医師不足，患者の低い満足度などの問題を抱え，英国人の誇りであるNHSが崩壊の危機にあったことは，まだ記憶に新しい．しかし2000年以降，“NHSを救う”とブレア政権下で実行された大胆な改革によって多くの問題点が改善され，現在，英国の医療は驚くほどの進歩を遂げている．
