現代日本の病院組織の特徴と経営課題—組織論とイノベーション経営の視点から

●米国病院組織研究では，病院組織は複数機能・目的をもつ高度に複雑な規範的ヒューマンサービス組織であり，高度な専門知識・技能・業務を独占し患者を統制する複数の専門職が，職種間・内で競争しつつ官僚制による統制と対抗する，「専門職官僚制」とされる．
●一般企業と比べ非競争的な患者市場とより競争的な労働市場，政府と職能団体による「弱い経営」のもとで，日本の病院組織は，専門経営者ではなく医療職が主導する専門職官僚制となり，組織の内部調整の時間・コストの増大，患者価値と労働者価値を向上させるイノベーションの抑制が懸念される．
●患者による価値決定が複雑・困難なサービスを提供する病院組織では，一般企業の経営技法の多くは有効性が限定されリスクも高い．イノベーションを促進するには，患者と病院職員が共同でより善い社会を探求・構築する，高度に複雑・創発的な組織が必要である．
