がん患者に対する苦痛のスクリーニングの現状—がん診療拠点病院等の指定要件に関する調査より

要旨
【目的】がん診療連携拠点病院等（以下，拠点病院）の指定要件に「がん患者の身体的苦痛や精神心理的苦痛，社会的な問題などのスクリーニングを診断時から外来及び病棟にて行うこと」が義務付けられている．しかし，明確なスクリーニング手法は定まっておらず，その質の施設間格差が指摘されている．拠点病院の指定要件に関する調査から，各施設のスクリーニングの手法やタイミングについての現状把握を行う．
【方法】全国の拠点病院を対象に実施した実態調査の中の患者の身体的・精神心理的苦痛や社会的問題のスクリーニングについて分析を行った．
結果：スクリーニングは多くの拠点病院で，入院時点までの早期に行われていた．一方，スタッフの人員不足や，特に外来での全がん患者の把握が困難といった問題点も明らかとなった．
【結論】スクリーニングの手法の妥当性評価や教育の充実など，さらなる改善が必要である．
