陰茎切断・陰茎再接着術の要点

要旨　陰茎切断は，稀な疾患で，自傷がその原因であることが多い。性器自傷は，精神医学領域の問題が背景となっており，最近では統合失調症に代わって，性同一障害などの人格障害が増加傾向にある。切断陰茎は，陰茎再接着術により救急的な救済が得られる。その制限時間は，24時間ともいわれているが，はっきりとした根拠はない。陰茎の血管神経系が再吻合されなくとも，勃起機能の温存が得られる報告が多いことから，陰茎深動脈の再吻合の必然性はいまだ示されていない。一方，皮膚や亀頭の欠損は，陰茎背動静脈再吻合で著明に改善される。
