前立腺小線源療法

要旨

 ヨウ素125を用いた小線源療法は低線量の放射性物質を前立腺内へ永久挿入する治療法であり,T2以下の限局性前立腺癌患者が適応となる。PSA 10ng/mリットル,Gleason score 6以下の症例は小線源単独治療となるが,それ以外には外照射の併用を行う。内分泌療法は前立腺容量が大きい症例に縮小目的として施行するのみで併用は行わない。治療成績は手術と同等とされるが,本治療の適応が早期癌であり比較はできない。他の治療法に比べて副作用は尿路・消化器症状ともに軽微で,特に性機能への影響は少ない。小線源の永久挿入による周囲への放射線被爆はきわめて少ないが,ガイドラインに準じた患者への適切な指導が必要である。
