腹圧性尿失禁の手術療法

要旨　女性腹圧性尿失禁の外科的治療は，腟前壁縫縮術から始まり，下腹部切開により直視下に膀胱頸部を挙上するBurch法などの開放手術から，より非侵襲的な経腟式膀胱頸部挙上術であるStamey法などの手術法を経て，現在はプロリンテープをスリングとして用い，尿道中部を支えるTVT（tension-free vaginal tape）法が主流となっている。このほか高齢者や重篤な合併症を持つ患者にも施行できるコラーゲン注入法があるが，再発による再治療が避けられない。本稿では，局所麻酔下でできる低侵襲手術として脚光を浴び，欧米のみならずわが国でも標準術式となりつつあるTVT法を中心に概説し，最近欧米で急速に増加しているTOT（trans-obturator tape）手術についても紹介する。
