腎盂尿管移行部狭窄症に対する経皮的内視鏡下腎盂切開術

　要旨：腎盂尿管移行部狭窄や上部尿管狭窄による上部尿路閉塞性病変に対しては開腹手術による腎盂形成術，minimally invasive therapyとしてのバルーン拡張術，腹腔鏡下腎盂形成術および内視鏡下腎盂切開術が行われている。内視鏡下腎盂切開術には，順行性に腎盂尿管移行部に到達する経皮的腎盂切開術と，逆行性に腎盂尿管移行部へ到達する経尿道的腎盂切開術がある。筆者らは経皮的腎瘻造設による順行性到達法により腎盂尿管移行部に達したのち，腎盂尿管移行部狭窄切開術を行っており，本稿では腎盂尿管移行部狭窄症に対する直視下切開手技，およびそれらを取り巻く問題点も含めて述べる。
