88．前立腺生検の目的，適応，および経直腸式，経会陰式それぞれの前立腺生検の特徴と標準的な施行方法について，また起こりうる合併症とそれぞれの対処法，予防的抗菌薬投与について教えて下さい。

1　前立腺生検の目的

　前立腺癌を疑って施行し，その確定診断をする。また，病理診断により悪性度を判定し予後の予測を行い，治療法を決定するのがその目的である。

　PSA検診などの普及により，最近ではT1c癌の割合が急増している。これは早期癌の発見率が上昇しているとともに，臨床上異議のない癌，いわゆるinsignificant cancerの発見や無駄な生検の増加をもたらした。したがって，適応の決定に当たっては根治的な治療が可能な癌の発見を増やし，insignificant cancerの発見を減らすべく考慮する必要がある。
