91．経尿道的膀胱生検の適応，方法，合併症について教えて下さい。

1　基本的事項

　膀胱生検で組織診断するに当たっては，壁内のどこまで組織採取する必要があるかを念頭に置く必要がある。膀胱腫瘍の診断においては，治療方針決定に深達度診断がきわめて重要である。すなわち，筋層内浸潤の有無の確認が組織採取においてなされなければならない。一方，尿路悪性腫瘍を疑った生検においても，局在が不明な尿細胞診陽性症例や，高異型度な表在性膀胱腫瘍を疑う隆起性病変における腫瘍以外の部位の生検は粘膜採取で十分である。腫瘍以外の診断目的で生検を行う症例では，基本的には膀胱粘膜の採取で十分である。

　ここで述べたいのは，単に膀胱生検といっても，何箇所採取するのか，深達度においてどこまで組織採取が必要なのかを常に考えて，安全かつ有用な診断情報を得られるように心掛ける必要があるということである。
