尿管結石の積極的治療方針―ガイドラインは医療の現場に何を指し示すのか？

　要旨：2002年に本邦の尿路結石症診療ガイドラインが編集されてから，本年末で早5年が経過する。このガイドラインに採択された文献のほとんどは，2000年までに掲載されたものである。その後，ESWL用の体外結石破砕機（lithotripter），TUL用の内視鏡と体内結石破砕機のそれぞれにおいて機器の進歩があり，治療結果や合併症，治療法の適応においても以前とは異なる報告がなされている。それに応じて，ガイドライン改訂の必要性も囁かれるようになってきた。今回は尿管結石の破砕治療について，私見をもって，最近の文献を元に，これまでに公表されたJUA，AUA，EAUの各ガイドラインを比較・検討してみたい。
