精索静脈瘤手術―顕微鏡下低位結紮術（鼠径管下到達法）

要旨：千葉大学泌尿器科では，精索静脈瘤に対して顕微鏡下低位結紮術を行っている。この術式ではより精巣に近い部位での結紮が可能であるとともに，顕微鏡下に確実に精巣動脈周囲の細静脈を処理できるため，再発率が低い。また顕微鏡下にリンパ管を温存することで，陰囊水腫などの合併症も少ない。鼠径管下到達法を用いていることで，より低侵襲となり，患者の早期社会復帰を可能とする。本稿では本術式の適応，要点，成績について概説する。
