急性腎不全

要旨　急性腎不全は，発症機序から腎前性，腎性，腎後性に分類されるが，早期に原因を確実に診断することが適切な初期治療につながり，ひいては予後を左右する。近年，高齢者に対しても侵襲性の高い検査や治療が積極的に行われ，急性尿細管壊死（ATN）の発症の頻度は増加している。ハイリスク患者がATNに罹患すれば，続発する合併症などにより予後は不良となる。ATNをきたす可能性がある患者には，検査や治療の開始前から十分な利尿をつけ，腎毒性薬物の使用を控え，脱水や血圧低下を防止することが肝要である。また，急性腎不全と診断すれば，素早く適切な治療を開始し，合併症の発生防止に重点をおいた全身管理を心掛けるべきである。
