47．再発を繰り返す表在性膀胱癌の患者です。9回目の経尿道的膀胱腫瘍切除術（TURBT）施行中に膀胱穿孔を起こし，内視鏡が腹腔内に入ってしまいました。まだ膀胱内に腫瘍は残っています。どのように対処すればよいでしょうか。

TURBT時の膀胱穿孔には，予定された手術手技として脂肪織まで切除するために穿孔を起こすcontrolled perforationと過度の切除のために誤って起こすuncontrolled perforationがある。一般的に膀胱穿孔といえば後者を指し，この症例も後者である。TURBT時の膀胱穿孔の頻度は5％未満とされている。Colladoら^1）は，2,821例中36例（1.3％）に膀胱穿孔を認め，そのうちこの症例のように腹腔内が6例（17％）で，全症例に対する割合では0.2％であった。また，Kondasら^2）のように，浸潤性膀胱癌に対するTURBT時の腹腔内膀胱穿孔の頻度は3％との報告もある。
