小腎腫瘍の画像診断―質的診断と術前評価

要旨　小腎腫瘍の鑑別診断と治療方針を決定するうえで，画像のチェックポイントを知ることが重要である。囊胞性腫瘤ではBosniak分類が非常に有用である。充実性腫瘤は，薄いスライス厚の単純CTでの脂肪の有無，発育形式，腫瘤の均一性，偽被膜の有無，造影パターン，T2強調像の信号強度などをチェックしていけば，不要な手術を避けることができるようになる。また，マルチスライスCTを用いたCT angiographyでは，腎動脈，腎静脈，腫瘍の位置などの評価がより小さい侵襲で可能になり，nephron sparing surgeryや腹腔鏡下手術の術前評価としての有用性が高い。
