腎癌における腹腔鏡手術の役割

要旨　腎細胞癌に対する外科的治療は1990年代までは開放手術で行われていたが，腹腔鏡手術の開発により大きく変化した。腹腔鏡下根治的腎摘除術はT1症例に限らずT2症例においても開放手術と遜色のない手術成績で，術後の患者の負担も少なく，低侵襲手術として標準的治療法となりつつある。腎部分切除についても腹腔鏡手術が行われており，直径4cmまでの突出型で，腎門部にない腫瘍にはよい適応となる。手技では阻血法と非阻血法があり，腎機能の温存をはかる。技術的には難易度が高いが，開放手術に比べ創部が小さく，患者の負担を減らすことができる。手技の向上や機器の開発により，腹腔鏡手術の適応の拡大が予想される。
