腎尿管全摘除術

要旨：上部尿路癌に対する腎尿管摘除術の原理は，病巣を取り残さないように臓器をen blocに摘出し，そして術後に尿溢流をきたさないように膀胱を縫合することである。これを成就するためには，（1）局所解剖を熟知し，必要かつ十分な術野を展開すること，（2）安全領域を認識し，かつ不測の事態に対処できる危機管理能力を備えておくこと，および（3）簡潔簡明な操作であること，が重要と考える。本稿では，われわれが行っている後腹膜式腎尿管摘除術＋膀胱カフ切除術を紹介した。
