下部尿路機能の基礎

要旨　過活動膀胱の病態を適切に把握するためには，下部尿路機能の基礎を十分に理解することが必須である。正常な蓄尿と排尿機能を維持するためには，膀胱・尿道の筋構築とそれを支配する神経機構が正常でなければならない。また，膀胱知覚の発現において，膀胱の尿路上皮細胞と粘膜下の知覚神経は相互に影響し合い，その機能的異常が尿意切迫感などの異常な膀胱知覚の出現に関与することが推測されている。ヒトの脳が下部尿路機能をどのように調節しているかについて，PETを用いた臨床研究により情報が収集されつつある。下部尿路機能障害の治療にあたっては，下部尿路の求心性，遠心性神経伝達の適切な理解が重要である。
