会陰式前立腺全摘除術

要旨：会陰式前立腺全摘除術（RPP）は，ミニマム創による恥骨後式前立腺全摘除術（RRP）や内視鏡下前立腺全摘除術（LRP）の普及した現在においても侵襲の低い術式である。多くの泌尿器科医にとっては馴染みのないアプローチのため限られた施設でしか行われていないのが現状であるが，根治性やQOLの観点からも恥骨後式と同等である。また，創部が小さいため術野の共有が困難な点はあるが，ビデオの活用などにより技術の習得は比較的容易で，他の術式と比較して習熟カーブも短い。今後，骨盤内リンパ節郭清を必要としないlow stageの前立腺癌がさらに増加すると予測され，低侵襲手術としてのRPPの普及が強く望まれる。
