EBMに基づく尿道下裂治療

要旨　尿道下裂の手術治療の基本は，陰茎の彎曲を是正することと，亀頭先端までの新尿道を形成することである。かつて，尿道下裂の陰茎腹側への彎曲の原因は，腹側に固着する索組織によるものと考えられていた。手術治療としては，索切除術と尿道形成術を時期を分けて行う二期的手術が中心であった。その後，尿道下裂の陰茎の解剖が解明されるに伴い，治療のコンセプトは変化し，一期的手術が趨勢となった。さらに，尿道板を温存した状態で陰茎の彎曲を是正し，尿道形成術が行われるようになった。一方，尿道板の離断を余儀なくされる尿道下裂も依然として存在する。また，非常に高度な症例には二期的手術の有用性が見直されている。
