新膀胱造設術の現状と展望

要旨　尿路変向術は泌尿器科手術の中でも最も重要な手術の1つである。尿路変向術の歴史は長く，その中でも自排尿型の代用膀胱形成術の登場は泌尿器科学史上最も大きな発展の1つといえる。近年開かれた尿路変向術の国際コンセンサスミーティングでは，長期的にみても新膀胱造設術の合併症の頻度は回腸導管に劣らないことが示された。一方，新膀胱造設術後の排尿機能に関してはおおむね満足できるものといえるが，夜間尿失禁などいまだ解決すべき問題も多い。QOL（quality of life）の評価に関しては当初予測されていた非失禁型尿路変向術との明らかな優劣はつけがたく，むしろ非失禁型尿路変向術患者においてもQOLは比較的良好に保たれていることが明らかにされた。今後は，排尿機能に特化した質問票の開発が望まれる。
