レーザー治療の現状と適応

要旨　良性疾患であるBPHの治療は，患者のQOL改善が目的であり，低侵襲でより安全な治療が望ましい。現在主流となっているlaser治療（holmium laser enucleation of the prostate：HoLEP，photo-selective vaporization of the prostate：PVP）は，いずれも従来のTURPと比較して出血が少なく，低Na血症の危険性が低いという利点を有する。核出術であるHoLEPは，大きなBPHに対しても比較的安全に行え，治療効果も安定しているが，手技がやや難しく習得に時間を要し，組織のモルセレーションにも問題が残る。一方，蒸散術の代表であるPVPは安全性が極めて高く，ハイリスク症例にも適応可能である。その手技取得も比較的容易であるが，蒸散効率の問題から100mlを超えるような大きなBPHに対する治療成績は確定されていない。
