77．精液に白血球が多く混じっている病態は妊孕性に影響があるのでしょうか。また，症状がなくとも治療すべきでしょうか。なかなか治らないのですが，どの程度の期間，抗生物質の投与を続けるべきでしょうか。

1　診療の概要

　精液に白血球が多く混じっている病態，いわゆる膿精液症の原因として，精路感染症や特発性のものがある。これら膿精液症は妊孕性に影響しないことも少なくないが，精子運動率が50％未満の，いわゆる無力精子症の原因となることがある。この場合，抗生剤治療によっても精液所見の改善がなければ，配偶者間人工受精（artificial insemination with hasband：AIH）や補助生殖技術（assisted reproductive technology：ART）の適応となる。

　精路感染症としては，慢性前立腺炎，精囊炎があるが，細菌感染が証明されるものは10％以下であり，多くは原因が特定できないのが現状である。
