要旨　閉塞性無精子症の治療には泌尿器科的治療として精路再建術が行われている。これはあくまで自然妊娠を目指してのものである。しかしARTの進歩，とりわけ1992年にICSIが始まってからは，挙児希望のカップルに対しての泌尿器科としての治療は変化してきている。受精に必要な精子の採取に，治療の必要性が変化してきている。このためにMESAあるいはTESEといった手技が重要性を増してきている。しかしこの手技には，染色体あるいは遺伝子異常の伝播といった危険性があることを知ったうえで行われるべき手技である。
