綜説

会陰式前立腺全摘除術アップデート

要旨　会陰式前立腺全摘除術に必要な解剖，手術手技，腫瘍学的および機能的アウトカムを概説した。会陰部の小切開で前立腺を摘除できる本術式は，恥骨後式や腹腔鏡下摘除術，ロボット支援手術の登場した現在でも，最も低侵襲なアプローチである。本術式は習熟カーブが短いうえに技術習得までの合併症が少なく，ダビンチのような高価な機器も必要ない。多くの泌尿器科医にとってなじみのないアプローチで，会陰式特有の合併症もあるが，適切な患者選択があればベネフィットは少なくない。本術式の習熟による解剖の理解は他のアプローチに対しても良い影響を与え，前立腺や尿道疾患の外科的治療にも価値ある技術や知識をもたらすものと考えられる。
