9．吻　合―尿管と腸管の吻合

要旨：尿路変向には，尿失禁型，尿禁制型，代用膀胱など，さまざまな方法があり，用いられる消化管も，胃，小腸，結腸と数が多い。尿路変向で一般的に用いられる吻合法は，尿管-小腸吻合と尿管-結腸吻合である。尿管-小腸端側吻合の代表がBricker法，LeDuc-Camey法であり，端々吻合の代表がWallace法である。尿管-結腸吻合法は端側吻合が主に用いられ，端側吻合にはrefluxing anastomosisとnonrefluxing anastomosisがある。前者の代表がNesbit法であり，後者の代表がLeadbetter-Clarke法，Goodwin法である。尿管-腸管吻合法において最も重要なポイントは，尿管の血流を維持することにより，尿管ならびに尿管-腸管吻合部狭窄を予防することである。
