16．膀胱全摘除術の適応と選択―尿路変向の選択の動向

要旨：WHOが1971年～2004年までに8施設で実施した7,129例の尿路変向選択の動向を分析したところ，47％の患者に自然排尿型の新膀胱，33％の患者に回腸導管，10％の患者にanal diversion，8％の患者に自己導尿型の新膀胱を造設していた。自然排尿型の新膀胱を造設する機会の増加に伴い，新膀胱造設術の適応，長期成績について欧米のガイドラインを参照にしながら考察を行った。数ある自然排尿型の新膀胱において，どの作成方法が最適なのか，現在も結論が得られていない。尿路変向の方法により，疾患特異生存率は変化しないものの，選択する腸管（結腸，小腸）によって術後の排尿状態が異なっており，その違いを認識して患者説明に反映させる必要がある。
