17．膀胱全摘除術の適応と選択―尿路変向の選択とその留意点

文献概要

要旨：膀胱全摘除術後の尿路変向術は，患者の生命予後や社会および家庭でのQOLに直結する重要な手術である。また尿路変向術の選択については，患者の希望を優先することが第一であり，膀胱癌の状態，それぞれの術式の長所および短所を患者および家族に十分に説明し，徹底した議論を行うことが必要である。特に，それぞれの方法の問題点を十分に理解していただく必要がある。本稿では，本邦で多く選択されている自然排尿型代用膀胱（新膀胱），回腸導管，尿管皮膚瘻を中心に，その選択のポイントを概説する。
