24．自排尿型―Studer法

要旨：Studer法による新膀胱は回腸を55cm遊離し，15cmを輸入脚に，40cmをパウチに使用する。40cmの回腸は脱管状化し，U字型に縫合して折りたたむことにより，ほぼ球形のパウチが作成される。輸入脚には，尿管は逆流防止機構を作らず，単純なend-to-side anastomosisにて吻合する。そのために，形状は単純で作成しやすい。新膀胱の機能を良好に保ち，患者QOLを維持するには，手術手技だけではなく，術前の患者の選択および術後のきめ細かいケアが重要である。ここでは，新膀胱作成のポイントとともに，術後のケアにもフォーカスを当てて解説する。
