32．代謝異常

要旨：腸管利用尿路変向術の主な合併症の1つである代謝異常をさらに細分化し，その病態と対策について検討した。血清電解質および酸塩基平衡異常は使用する腸管によりタイプが異なり，それぞれの腸管特有の病態生理に対しての理解と適切な対処が必要である。そのほかに，アンモニア代謝の変化が主な原因である感覚異常や，腸管より吸収され腎で未代謝のまま排泄される薬剤の代謝異常，アシドーシスとビタミンD抵抗性が関与する骨軟化症，小児症例に対する成長と発達への影響，尿路感染症や尿路結石の発現の増加，栄養問題として水分や電解質に加え，胆汁酸，脂肪，ビタミン各種の異常，癌の発生率の上昇などについても，同様に理解と対策を講じる必要がある。
