癌患者の精神症状緩和―サイコオンコロジー

要旨　癌の病期にかかわらず，臨床的に介入あるいはケアが望まれる精神症状はおおむね半数の患者に認められ，中でも適応障害，うつ病，せん妄の頻度が高いことが示されている。これらの精神症状は，患者のQOL低下，治療アドヒアランス低下，家族の精神的負担の増大，自殺，入院期間の長期化など，多岐にわたる問題に影響を与え得ることが明らかにされている。一方では，これら癌患者の経験する精神症状の多くが看過され，適切な医療が提供されていないことが繰り返し報告されている。本稿では，これら癌患者に頻度の高い精神症状の診断，治療を中心に概説した。
