小腎腫瘤の鑑別診断―腎腫瘍の画像診断のポイント

要旨　囊胞性腎腫瘍の鑑別は，Bosniak分類が非常に有用である。①囊胞壁や隔壁の厚さと不整度，②石灰化の程度，③内容液の性状，④増強効果の有無，がチェックポイントであり，無治療・経過観察・手術の治療方針に分けていく。充実性腎腫瘍は，膨張性発育型と浸潤性発育型に分けて考え，脂肪の有無，腫瘤の均一性，偽被膜の有無，造影パターン，T2強調像の信号強度，がチェックポイントである。充実性腫瘍では，経過観察，生検，手術の治療方針に分けていく。これらを用いた現在の画像診断の鑑別の可能性と限界を述べた。
